The Immunology Of Kidney Transplantation And The Evaluation Of The Immunological Matching Between Recipient And Donor

aNefroloji Ünitesi, Hacettepe Üniversitesi Tıp Fakültesi, ANKARA Renal transplantion (Tx) is the best treatment of the end stage kidney disease. The immuno logical matching in kidney Tx is achieved by the similarity of the HLA and ABO blood antigen systems between the donor and the recipient. The probability of success decreases as the mismatch in these two systems increases. An unsuccessfull Tx, on the other hand, renders the patient sensitive for the next one.
There are also non-immune factors (e.g. donor’s age, the cold ischemia time of the graft kidney, the endowment of the donor, etc.) which are as important as the immune ones. Indeed, there are cases where, despite lacking immune matching, the kindey function tests are normal. The successful results in these latter cases, the fact that the number of recipient patients is much more higher than that of the donors and the high cost and low-qualıty of the dialysis life are the reasons compelling kidney Tx in the face of ABO&HLA antigen systems mismatch between the donor and the recipient. Today, the renal Tx for these patients are done by several immunosuppressive methods. However, those patients graft kidney survival is short and the treatment of those patients is more expensive com pared to the ABO&HLA matching systems.Keywords: Kidney transplantation, LY96 protein, humanTurkiye Klinikleri J Nephrol-Special Topics 2008, 1:53-58
